HSA Custodial Agreement

This document explains the official rules for how your Health Savings Account (HSA) is managed by DataPath Financial Services. It covers what the company (called the “custodian”) is allowed to do – like holding your money, keeping records, and making payments when you request them. It also explains your responsibilities, like making sure you’re eligible for an HSA, keeping track of your contributions and expenses, and following tax rules.

The agreement also talks about things like how to use your HSA debit card, what happens if you pass away, how fees are handled, and how investments work if you choose to invest part of your HSA balance. It includes rules for resolving disputes, making changes to the account, and protecting your privacy.
